Step 2:

(or download or print and email) If you do not have time to complete the online application all at once, feel free to download the application and complete on your own time. Please write your responses legibly.

When you are finished, save your document and email the form back to us at (Pahinstructor1@gmail.com)

Step 3:

Once we receive your completed application, we will reach out to you to schedule a day and time for you to come to our office to complete the application process.

Here is the most exciting part……Once all of the below information is reviewed and received, you will be granted access to the next Training Course.

  • Completed Student Application 

  • Submit TB skin test

  • Submit shot records

  • Submit Covid Vaccination card if applicable

  • Submit High School/GED transcript

  • $25 Non-Refundable Application Fee

  • Criminal Background Check

  • School Tour

  • Full Payment of Tuition, Voucher from School Partner or Payment Plan Arrangement
